My Plan

My Plan is a plan each individual, along with the team of his or her choosing, constructs to capture his or her goals. The individual and his or her team will design services and supports based on the person’s strengths, needs, cultural background, preferences, and the outcomes he or she wants.  The My Plan is a fluid document that can be adapted at any time to reflect a person’s needs.
